Installation Forms 4. TRUNK CARD SWITCH ASSIGNMENT FORMS General 4.01 The following forms enable the installer to configure the switches of the system’s Trunk Cards. There are switch assign- ments for CO Trunk Cards, E&M/Tie Trunk Cards and DID/Tie Trunk Cards. These forms complement Form 13 - Trunk Circuit Descriptors (refer to Table 3-13, in this Section).
